My advice is, get a (skilled) programmer and a separate (skilled) designer. It's extremely rare to find someone at a high-level who can do both those tasks in one, so likely you'd have a great programmer who can use Photoshop a bit or a great Designer who knows basic PHP; which isn't what you need.
