Short form can say anything you want it to.
I had to go get my Son's SS card replaced, they asked for my moms Maiden name and place of birth. I got BOTH questions wrong, since I gave them the info on the Short form.

It's because Hawaii didn't become a part of the Union until 1959. So all people on the soil of Hawaii were issued legal name birth certificates because no one could "prove" live birth for a CLB.
The short form is how all things legal are handled so it in reality all one would need. That is unless you want to be, say, President.. Before Obama anyway.
The only person alive that could verify (that we know of) where he was born was his grandmother. She unfortunately died during his campaign but not before saying he was born in Kenya.
