I am having the hardest time trying to get an ACH authorization form approved for a new bank account.

I have submitted a bank statement, my driver's license, and the ACH form all with identical matching information but the ACH form keeps geting rejected. All I get in the rejection email is a generic "information does not match" but it does not state what does not match so I can fix it.
