If they mean it because they hate someone for being born a certain way.
For example, the word fag doesn't bother me unless there is hatred in the one saying it. If one of my best straight friends says you're such a fag because I do something ridiculous and over the top, they obviously aren't homophobic or they wouldn't be one of my best friends.
