I fully agree with johnclark, but if you use some automation, then it's hard to get good handwritten-like alt/title tags. If you use Wordpress then take a look at
SEO Friendly Images and you can easily use templates like "Hot pics of Holly featuring artporn, fucking, sex..."
Second variant will raise a red flag for keyword stuffing.
