On page? Your own hands. Just modify head.php of your theme to make it do whatever you want and how you want it. There is no plugin which can do that in a proper way, because you have to modify the theme, but not to try to fix PHP output on the fly.
But to be honest, that's not a SEO at all. Your page content and a user experience is the key. So if you want to perform some automatic SEO optimization, look at the tools that modify your content, not post titles and meta tags. The tools that modify and improve it. The tools that make it unique. The tools that catch a surfer who accidentally visited your site, to glue him and don't let him go. That's all Google spying services are expecting to see when they rate your resource.
