How do you buy the top actual spots on google search results?

I've bought square text based ads through adwords many times, but how do you buy the actual placement? The pseudo real looking "results" which are highlighted..

Will they sell me the traffic even if there's no competition? I had a hell of a time with them cancelling good effective long tail terms before, because they "weren't active enough" or some garbage reason

you max cpc and quality score have to be high enough to show. plus you have to setup your ads to meet their requirements to show headline + description line 1 all in 1 line.

How does that work?
I would always have a linked headline with what two more lines of desc and then the base url on another line... looks like they're still the same, now?

Is "quality score" still based on the CTR? To me, them pulling ads with low CTR was a ripoff. I always wanted my ads to ONLY be clicked by someone ready to BUY. Well, of course, they don't make as much money showing ads that aren't clicked. So "quality" was relative not to "actual quality" but to "quality to their revenue base"

Well. Today I woke up, went over to a friends house to fix his computer and they had the best fried shrimp and chicken and roast beef in the world... Not to mentioned the real vine ripened tomatoes, and a good cigar

Then I had a nice 45 minute night drive, this evening... twice. Love those.
