So I'm setting up a hentai site but I'm not sure if it would better to leave it as regular hentai with no categories. Or add in shemale/gay hentai content and separate it with categories.

However there's no traffic niche for gay hentai so I'm guessing I'd be better off leaving it as normal hentai content?

Has anyone tried combining both straight/gay in 1 site with good results?

Add shemale. That is a good cross over for straight guys. Maybe toss in some bi-sex and test it out, but I wouldn't go full gay.
