US navy men died in flooded compartments found on the Fitzgerald

There were 285 crew onboard, although, I´m not sure how many died in this tragic incident

The bodies of missing sailors were found in flooded compartments of the USS Fitzgerald, which came close to sinking after a collision with a container ship off Japan tore a gash under the warship's waterline, the U.S. Navy's Seventh Fleet commander said on Sunday.

The container ship has a 29,000 ton displacement, & the U.S warship´s 8,315-ton. This is a terrible tragedy that must come from human error, someone will be reprimanded & lose titles for this. How can something like this happen?
