Epoch is great for throughput, ccbill is good because everyone is already a member of their affiliate system - that's my experience. I haven't used any others as a merchant.

For my money (well, it is my money) I'm very satisfied with commercegate. I like it when people actually answer the phone when I have to call them.

I think Epoch is the best one these days.
