How to fix 404 errors in GWT?
 
Since last month, I see a huge increase of 404 errors in my website that doesn't make sense.

For example:

Any idea of what could be?

It seems that is grabbing the image title and put in front of the url:

Any idea of what could be and how to fix it?

same here -- just updated an all the image paths are fucked up (broken -404) on the pages clicked (requested) from the menu with similar URI paths in the source code.

Another wordpress success story ...

Is your server also Niginx with a PHP 7 gateway?

I think my issue is different. All my images are working fine.
The problem is that somehow, WordPress are using image names to create inexistente urls.

Is that your issue too?
Can you email me a list of the plugins that are active in your installation?
Maybe is a plugin problem.

I think I have found the problem. I changed accidentally my Permalink Custom Structure a few months ago and only notice a few days later.
Probably google was crawling my website during that time.
It seems that the errors are starting to disappear now.

I am not using any plugins.
I am just messing around with WordPress for an SEO experiment.
