Question for all L.A webmasters/self employed people

Are all Los Angeles based affiliates required to fill out this form if they make over $5k a year via 1099's?

And if so, what to put under "total amount of contract" and "valuation of work" if we don't know? How can we know this if the year isn't even over yet?

I used to live in OC we never had to fill this out. Is this exclusive to L.A?

you must pay an LA city tax, in advance each year. you will get no rebate if you make less than stated. if you make more, they will come after you.

But most self employed people have no idea how much money they are going to make. Are we supposed to guess?

What if we end up getting a big job and get an unexpected amount of money?

When I lived in Orange County we didn't have city tax, only state and federal.

the tax applies to self employed people. ues last year as a guide. los angles isn't orang county
